NIOSH - Control of Smoke From Laser/Electric Surgical Procedures
"During surgical procedures using a laser or electrosurgical unit, the thermal destruction of tissue creates a smoke byproduct. Research studies have confirmed that this smoke plume can contain toxic gases and vapors such as benzene, hydrogen cyanide, and formaldehyde, bioaerosols, dead and live cellular material (including blood fragments), and viruses. At high concentrations the smoke causes ocular and upper respiratory tract irritation in health care personnel, and creates visual problems for the surgeon. The smoke has unpleasant odors and has been shown to have mutagenic potential."

Les lasers dermatologiques fonctionnent énormément sur le principe de phothermolyse d'un chromophore spécifique. Il y a donc échauffement et parfois compétition entre les chromophores dont ceux de la peau. On cherche en épilation laser, par exemple, à détruire la mélanine et chauffer la base du poil, tout en limitant l'échauffement de la peau. Divers systèmes de refroidissements sont utilisés: la pulvérisation de spray de gaz frigorifique, le froid pulsé, le froid par contact avec un saphir (lampes flash) ou par circulation de liquide (effet peltier).
Certains patients, pourtant compatibles avec le traitement laser, peuvent présenter des contre indications, simplement au froid et donc ne pas pouvoir être traités ou nécessiter des précautions.
Les contre indications à la réfrigération de la peau sont:
- Allergie au froid
- Engelures locales
- Syndrome de Raynaud
- Troubles de la circulation artérielle
- Plaies ouvertes ou infectées (risque de dissémination de l'infection par soufflerie)
- Cryoglobulinémie
- Anémie auto-immune hémolytique

 L'utilisation de lasers nécessite une série de précautions de plusieurs natures: précautions éléctriques, précautions anti-incendie, précaution pour préserver le laser de l'usure, précaution pour ne pas détruire le laser par mauvaise manipulation, précaution pour ne pas nuire à l'opérateur et précautions pour ne pas nuire au patient. Tous ces points font l'objet de procédures de qualité qu'il est bon de poser par écrit et de consigner dans un cahier de procédures consultable par tout le personnel travaillant en milieu médical laser.
